I'm sure many of you have thought about it, I have, a couple of 100 times a day, and there's nothing wrong about it. At the time your parents had you, they were probably excited and also they never thought their innocent child would end up like you are now.
As we grow older some of us start to realize how unsuitable our names are with our character and personality. It is just not a vibe, we're all emo and gothic and our names be Princess or something silly like that.
Or some of us have serious problems like our names are misspelt or we changed our religion and now have a new name.
Or you know, just the usual, don't like your name thingy...
Well, chuck your worries in a river because 2022 is coming and it is time for a CHANGE. Time to change your name, LEGALLY!
Here's how to do it:
1. For parents who want to change their child's names
You will need:
+ MyKad of both the parents (original and copy)
+ parents’ wedding certificate
+ child’s birth certificate
+ child’s MyKid (if there is one)
+ A good reason why you're changing their name, if the officer asks
Go to the Jabatan Pendaftaran Surat Lahir in the JPN office, complete the JPN.LM21 form and get the JPN LM07 form and then go to a Commissioner of Oaths with a copy of your forms. Get their stamps on your forms and then submit everything to the JPN office.
You will need to pay RM5 for the a new birth certificate and another RM5 for the processing fee.
If the child involved doesn't have both birth parents, then a legal guardian can so the name change, just bring the original copy and a photostat copy of the legal guardianship letter issued by Jabatan Kebajikan Malaysia.
2. Name change because you don't like your name
You can do it but if asked, you'll need to have a good reason as to why you want to change your name.
To begin the process, you can start by signing a statutory declaration in the presence of a Commissioner of Oaths and get their stamp.
Bring your original MyKad and a copy of it and head to the JPN office to fill up the JPN.KP16 form. There may be some other forms or documents needed, depending on why you’re changing your name.
3. Changing for religious reasons
Just like the above point, you’ll need your MyKad, the JPN.KP16 form and only one or two more extra forms/documents that may apply to you.
Besides that, since you're changing your name for religious reasons then, you'll need proof of your conversion. For Muslims, you’ll need a card that proves your new religion from the Islamic Religious Department or State Islamic Religious Council.
For other religions, bring along proof of baptism or other documentation that proves conversion.
